Adena Sports Medicine will provide FREE physicals for Zane Trace Athletes on Tuesday, May 14, 2013.
8th Grade Athletes - 8:00 am - High School Gym
High School Athletes - 9:00 am - High School Gym
(Students will be dismissed from class by the PA system)
Pre-participation Physical Evaluation and OHSAA Authorization Form below. This is a requirement from the Ohio High School Athletic Association. Please make sure the student athlete and the parent sign at the appropriate places on each form. (4 of 6 pages must be signed). All information must be completed in order to try out for a team.
You must have the completed forms on the day of the Physicals.
